Decode and encode underscores for header style encoding. Fixes bug #463996.

add some description of what the quotetabs argument does for the
encode*() functions.  Finally, add a "see also" pointing to the base64
module.

macros.

checkin of myformat.sty.

Change "\renewcommand{\indexsubitem}{(...)}" to "\setindexsubitem{(...)}"
everywhere.
