parameters (like \UNIX) are commonly entered using an empty group to
separate the markup from a following inter-word space; this is not
needed when the next character is punctuation, or the markup is the
last thing in the enclosing group.  These cases were marked
inconsistently; the empty group is now *only* used when needed.

This has sat around in a deprecated state for a *long* time!

elsewhere in the library reference.

Fix up a few synopses.

Changed example to use "import posixfile" instead of "from posixfile import *"
since only one symbol was used.

checkin of myformat.sty.

Change "\renewcommand{\indexsubitem}{(...)}" to "\setindexsubitem{(...)}"
everywhere.

Some other minor nits that I happened to come across.

two-column tables.  This improves the rendering of the HTML version on
(at least) some browsers.

in posixfile (which is obsolescent).

 unified style; etc.

lib.tex: add libimp; remove bogus warning about lineii.

libmd5.tex: rename md5.md5() to md5.new().

libposixfile.tex: openfile() instead of fileopen().

ref7.tex: don't use \verb in footnote.
