Fix up a few synopses.

(new!) contains.

Combine function pairs:  add() and __add__() go together.  This way they
share a single description and use less space.

checkin of myformat.sty.

Change "\renewcommand{\indexsubitem}{(...)}" to "\setindexsubitem{(...)}"
everywhere.

Some other minor nits that I happened to come across.

Lots of little nits:  parameter names in text should be \var{}, etc.

	* \bcode, \ecode added everywhere
	* \label{module-foo} added everywhere
	* A few \seealso sections added.
	* Indentation fixed inside verbatim in lib*tex files
