sw: prefix members of SwFont

Also de-obfuscate a few variable names:
- nToxCnt,m_nToxCount (one would wonder if content or count)
- nRefCnt,m_nRefCount
- bFntChg,m_bFontChg
- bNoColReplace,m_bNoColorReplace (one would wonder if color or column)
