- m_oldTableReference values were not being reset.
- A call for DocxAttributeOutput::switchHeaderFooter from
  DocxExport::WriteHeadersFooters would end up using the m_oldTableReference
  value saved by DocxAttributeOutput::EndParagraph, which is incorrect.
- Fixed this issue and added an UT for the same.
